Certificates Of Origin

The Malta Chamber is authorised to process Certificates of Origin and to authenticate other documentation.

The standard forms for a Certificate of Origin are available on request from the Secretariat. The following are required in order to process Certificates of Origin: 

Authorized Signature Form: It is important that the authorised signature form is completed by persons authorised to sign documentation on behalf of the applicant company and returned to the Chamber together with a photocopy of their I.D.. Card or passport.

Application Form for a Certificate of Origin: The authorised person must sign the Application Form for a Certificate of Origin. This is retained by the Chamber Secretariat.

Certificate Back Page: The Certificate Back Page contains instructions for the completion of the forms and must be printed on the back of the Chamber Copy.

Customer Copy: The Customer Copy is the original Certificate of Origin and must be signed by a Malta Chamber official for it to be certified. Unless otherwise requested only one original will be given to the customer.

 Supporting documents are required for goods which are not of Maltese origin. Appropriate supporting documents are a Supplier’s Invoice or Certificate of Origin which has been stamped and signed, showing the origin of the goods.

Authorisation is required in cases when an agent is issuing a certificate on behalf of another company.
